The doctors and staff at Major Oak Medical practice in Edwinstowe, Mansfield are proud to offer the highest standard of patient-centered healthcare.
We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

Emergency and Out of Hours
“In the event that the surgery is closed you can seek advice by phoning NHS 111 for non-emergency symptoms, or by contacting your pharmacist. Calls to the NHS 111 service are free from both landlines and mobiles. If these services suggest you need to seek urgent care then Primary Care 24, right next to the Emergency Department at King's Mill Hospital, provides a 24 hour walk in service.
Out of Hours is from when the GP Surgery closes to the time it opens in the morning. If you require urgent attention during these hours, appointments are made through 111 and you will be seen at your local Out of Hours Service provider.”

Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency. Stroke symptoms (face fallen on side, loss of movement in one arm, speech slurred) constitute an emergency. Accountable / Named GP for All Patients
From 1 April 2015, practices are required under the contract to allocate a named, accountable GP to all patients (including children).
Individual patients will be informed of their named accountable GP at the first appropriate interaction with the practice.
Your named accountable GP will be the doctor with whom you are registered.
If you wished to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not always be available and, if your needs are urgent, you may need to discuss them with an alternative doctor.
However, the practice does keep a record of your registered (accountable) GP. If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
Please note: there is no need to telephone the practice for this information.
Where a patient expresses a preference as to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request.
OVER 75 HEALTH CHECKS
All patients over 75 who do not already attend the surgery for an annual review are welcome to book a health check with the health care assistant. The health care assistant will check your blood pressure, weight, height, urine and enquire about any health needs. Please book with the receptionist if you wish to have a health check.
This should promote good general health and can help to detect minor problems that you have not had a chance to discuss during a regular consultation.

CQC INSPECTION REPORT
As from 2nd July 2013 we be starting to use the Electronic Prescription Service...
The Electronic Prescription Service allows your prescription to be sent electronically to a pharmacy of your choice.
To sign up, you need to speak to your preferred pharmacy and complete a form to confirm that they can receive your prescription directly.
Once you have signed up, your chosen pharmacy will receive your prescription electronically and you will not need to collect your paper prescription from us. If you have already nominated a pharmacy, from 02/07/13 your prescription will be sent electronically to them unless you tell us otherwise.
Should you need to obtain a paper prescription for any reason, just let us know at the time of ordering. This service is optional and you can continue to collect paper prescriptions as you do now if you prefer.

In a survey conducted over 3 months 52% of the 383 patients who responded said they would recommend our surgery.

The practice is continuing in its search for a salaried GP. In the meantime we have permanent locum doctors in place to ensure continuity of care. Unfortunately, it is not always possible for the clinician(s) to run on time due to the unpredictable nature of the job. We do ask that patients wishing to present with more than one problem to inform the reception at the time of booking so that a double appointment can be given, however this is not always the case. Clinicians are aware that running late may be inconvenient to the patients who are waiting, but depending on individual circumstances, it is not always possible to rush a consultation as patient care is of paramount importance to us. Every attempt is made to ensure that clinics run on time and catchup slots have been introduced in an attempt to improve the situation.
Please accept our apologies in the event of a clinician running late.

The NHS Friends & Family Test
We want you to have the best possible experience of care. The NHS Friends and Family Test is a way of gathering your feedback, so we can continually review our service. It is based on one simple question:

Your feedback will help us learn more about what you think of your expeirence - what you like and what you think we could improve. Ultimately, you're helping us to make changes that will ensure we can offer the best possible care.

Patient Participation Group
We have a successful Patient Participation Group that is up and running who would welcome any other patients to work with them in a 'virtual' form. If you are interested in helping to make improvements to the practice and are happy to partake in future surveys, etc, please click on the 'Join our Patient Group' tab.
We look forward to improving services and communication to our patients through this group.
The results of the practice survey are posted on the website and can be located in either the Patient Group or Survey Report area which can be selected to by the link on the right hand side of this page. Paper copies are also in the practice.
Nurse Triage - our Senior practice nurse Sarah Townsley, who passed her prescribing course will have daily traige clinics booked to see patients. All patients calling the surgery requesting an appointment for that day will be contact by Sarah before 10.30am, Sarah will take a history of the medical complaint and book an appointment with the most appropriate clinician.
Minor Injuries Clinics - Our qualified Nursing team are happy to provide a Minor Injuries service to our patients should you need it. Please contact the surgery first, rather than attending Accident and Emergency at the hospital, so that we can try to assist you in the best way possible.
Free Bleed Clinics - Will run Monday to Friday 10:00am - 10:30am. Please note that any other issues, other than blood tests can not be dealt with in this clinic and a further appointment will need to be booked.
Online Appointments / Prescription Ordering
We now have appointments available with doctors to book online and the facility to order your monthly repeat prescription online. Please register at the surgery for this facility. You will need to come in and collect your personal username and password to use the online facility, unfortunately, we cannot give this to anyone or post this to you due to security reasons.
